    1. Personal Information

Please attach a copy of your resumé.
    Max file size: 20MB

    2. Flexibility

TUEFRT has two different shift types during an average week. The day shift starts at 0850 and ends at 1650 (8hrs) and the night shift begins at 1650 and runs until 0850 (16hrs). During a shift, responders are able to attend lectures however scheduling yourself to be on call during a seminar or laboratory time slot may result in you missing valuable course content should you be dispatched to a call. Thus, for the availability we ask that you only provide the quantity of time during a 7 day week that you will be able to take a shift either on free time or lecture time, excluding any time that you are scheduled to be at a seminar or laboratory as we will not allow you to take shifts during that time.

    3. Questionnaire

Please answer ALL of the following application questions to the best of your ability ensuring that your responses are no greater than 250 words in length.

    4. Declaration
